Respirator Examination with Pulmonary Function Test
Respirator exam includes a Pulmonary Function Test and a fit test. Employee will need to bring in respirator and be clean shaven
Employee will need to provide OSOS with a return to work clearance note from their MD. This Eval is utilized to ensure an employee is physically able to provide work functions after a medical leave.
Return to Work Medical Evaluation with a urine NON-DOT drug screen. Employee will need to bring in medical clearance note from their provider.
Fitness to work evaluations are conducted when an employer has noted physical signs, or symptoms that may indicate that the employee may have a medical condition that would impair their ability to perform work functions. Occ Safety will need documentation of the employers concerns prior to appointment
Asbestos Physical Examination- Includes a pulmonary function test and a chest X-ray at baseline and if clinically indicated.
Hepatitis Vaccine- Series of three injections over a six month time frame
